What Are My Implant Options?
The technology used in breast augmentation has come a long way from the first procedures. Today, there are two unique implant options:
Saline-Filled Breast Implants
These implants are filled with sterile saltwater (normal saline). They cost less than other types of implants, and when they eventually wear out, it’s easy to diagnose implant failure and replace them.
Saline breast implants are filled at the time of placement, which allows for minor modifications in size once the surgeon can actually see them in the body. The downside to saline implants is that they’re slightly heavier, denser devices with more rippling. They also have a valve that can leak and generally feel firmer, much like the bags of saltwater they are!
Silicone-Filled Breast Implants
These implants are filled with a soft silicone gel that feels more like natural breast tissue. Gel devices are softer, slightly lighter, have less rippling, and don’t have any valves (significantly reducing the chance of leakage). As such, they come pre-filled and cannot be added to or subtracted from.
Today’s third and fourth-generation silicone breast implants are cohesive, which means the gel is more rubberized. It’s also thicker to maintain the shape of the implant better. On the other hand, when silicone implants wear out, it’s harder to diagnose an implant malfunction.
Gel implants also come in many different sizes. The best size implants for a patient are the ones that fit with her frame.
Additionally, implants come in different profiles. Low, moderate, and high-profile breast implants are all available depending on the patient’s needs and goals.
What Are My Placement and Incision Options?
There are two placement sites and three incision locations. Breast implants can be inserted below the major pectoral muscle (submuscular or subpectoral) or on top of the muscle (subglandular or subfascial). Putting the implant beneath the pectoral muscle may be useful for patients with minimal soft tissue coverage, thereby hiding possible rippling.
Dr. Rottler will help you determine which option is best for you during your breast enlargement consultation. He will also help you decide on an incision location. The three main choices are:
- Inframammary: The incision is made underneath the breasts, right above the crease.
- Periareolar: The incision is made around the lower area of the areola.
- Transaxillary: An incision is made near the armpit.
Am I an Ideal Candidate?
Typical candidates for breast enlargement are physically healthy, have fully developed breasts, and have reasonable expectations.
Ideal patients seek out this procedure for any of the following reasons:
- Unhappiness with breast size
- Loss of breast volume and shape after pregnancy, aging, or weight loss
- Upper area of the breast seeming “empty”
- Asymmetrical appearance
- Elongated breast shape

Many patients benefit from a combination procedure called breast augmentation with lift. Technically referred to as augmentation mastopexy, it combines a breast lift and implants to enlarge the breasts while eliminating sagging. If you want a lift without an increase in size, a breast lift without implants is the better option.
Your Consultation
When you come to your consultation with Dr. Rottler, be prepared to discuss your desires and expectations, as well as your medical and family history. A nurse will take photos of your breasts for medical records and examine and measure your breasts, nipples, and areolas. Examining the size, shape, skin quality, and placement will allow for the best recommendations for your procedure.

How to Prepare For Breast Augmentation Surgery
Breast augmentation procedures require proper preparation to be safe and achieve optimal results. The following are some general instructions to follow before breast augmentation surgery:
- Prepare your home for the recovery process – You will be physically limited while healing. Because of this, it’s important to make all recovery essentials easily accessible in advance.
- Clear your schedule – Arrange for time off from work. Additionally, cancel any plans for strenuous physical activities like sports or heavy lifting.
- Arrange for assistance – You’ll need someone to help you with your daily tasks at the start of recovery. Ask a friend, family member, or healthcare professional to assist you.
- Avoid smoking and alcohol – Do not smoke for at least six weeks before and after the date of surgery. Additionally, don’t drink alcohol several days before the procedure. These activities can increase the risk of surgical complications.
The Breast Augmentation Procedure
The surgery itself takes between one and two hours, during which time you’ll be under general anesthesia. The anesthesia will be performed by a board-certified anesthesiologist who will call you the night before the operation to have a pre-consultation with you. General anesthesia means that you will be “asleep” and won’t feel anything.
Dr. Rottler will then make an incision under your breasts, around your areolas, or in the armpit, depending on the decision made during your consultation. They will then create a pocket in the breast and place the implant in that pocket. After that, Dr. Rottler will close the incision with dissolvable sutures.
What is Recovery Like?
Immediately following your surgery, you will be taken into a recovery room to be closely monitored. The incision locations will be dressed in surgical gauze and you’ll be placed in a bra that will support the breasts as they heal, as well as minimize swelling. You may have to wear this bra for the first month following surgery.
Most of your discomfort or pain will be experienced during the first 48 hours immediately after surgery. Your breasts may feel sensitive or tight, and there will be some swelling, discoloration, or bruising, but these concerns should disappear within the first couple of weeks. You will have pain medication to deal with your discomfort.
It is also crucial to follow your surgeon’s instructions regarding applying topical antibiotic ointments and cleaning the incision sites. Other dos and don’ts include:
- Do get plenty of rest.
- Do sleep on your back.
- Do eat properly.
- Do take prescribed pain medications as directed.
- Do stay patient.
- Do contact your surgeon if you have any concerns.
- Don’t perform any heavy lifting.
- Don’t take a bath or swim in swimming pools, bathtubs, jacuzzis, lakes, rivers, or seas.
- Don’t smoke.
- Don’t sleep on your stomach.
Dr. Rottler will advise you on when you can return to your normal activities. You should be able to walk immediately after surgery, which is actually crucial for preventing blood clots. Refrain from any lifting, pushing, or pulling during the two weeks after your breast augmentation. In general, you’ll want to be extremely careful with your breasts for at least the first month.
